This page contains information regarding medical registration including:
- A letter containing a link to the NHS Family Doctor Services Registration Form (GMS1). There are two versions of this letter, one for home student and another for international students.
- There is also a link to a confidential health questionnaire which needs to be completed.
- A letter for your current GP to enable you to get the appropriate vaccinations before you arrive.
The completed GMS1 registration forms and health questionnaires should be sent by email or post to the Claypath & University Medical Group by Friday 20 September 2019. To register with other local medical practices you should complete the (NHS) Family Doctor Services Registration Form (GMS1) and on arrival in Durham take the form to the medical practice in person with a form of photo id and proof of address.
For international students, there will be an opportunity on Friday 27 September, to speak to the Claypath & University Medical Group on NHS services, how to access appropriate treatment, medical registration, vaccinations, pharmacies, emergency treatment, contraception and general health issues.
