Referral to Occupational Health

For Managers who need to refer a member of staff to Occupational Health. Please see the Management Referral section https://www.dur.ac.uk/hr/occupationalhealth/managementreferrals/for further information.

Update Referral to Occupational Health

This form is to be completed when the Referring Manager or HR have updates following an initial referral. The staff member must have been seen by Occupational Health in the last 6 months for the same condition. Eyesight Test Claim Form

Must be completed by the Optician and staff member in order to make a claim. Please see the DSE assessment sectionhttps: //www.dur.ac.uk/resources/hr/occupationalhealth/DSEformSeptember2019-Version001.docx for further information.

First Aid Authority Form If you are attending a 3 day first aid course Occupational Health require this form to be completed by the budget holder 7 days prior to the training. Refresher Authority Form
