Referral to Occupational Health

Eyesight Test Claim Form

Must be completed by the Optician and staff member in order to make a claim. Please see the DSE assessment sectionhttps://www.dur.ac.uk/hr/occupationalhealth/displayscreenequip/for further information.

First Aid Authority Form If you are attending a 3 day first aid course Occupational Health require this form to be completed by the budget holder 7 days prior to the training. Refresher Authority Form
