The information provided on this form will be used by Durham University to make contact with you to complete the registration process and administer your child's place on the Team Durham Holiday Camps. In applying for a place on this programme and by submitting this application form, you consent to your child's personal data being held and processed by Durham University. Parental Consent
To be agreed by parent or guardian:
I understand and hereby agree with the following statements:
(a) No liability shall arise or occur against Durham University for any loss, damage or theft of personal property belonging to said child, whilst participating in any Durham University activity.
(b) No liability shall arise or occur against Durham University for any accident, injury or death occurring whilst participating in Durham University activity, except to the extent that such injury or death was the result of a negligent act or omission of Durham University.
(c) All information provided is true and accurate and the above child is in good physical condition and capable of engaging in the level of exercise associated with sporting activity. Exercise will not be detrimental to said child's health, safety, comfort or physical condition.
(d) If said child's health status/ medication changes whilst engaged with the project I shall notify staff immediately.

Media Consent:
Occasionally, we may take photographs of the children during activities and events. We may use these images in printed media produced by Durham University and/or other stakeholders, such as prospectuses, guides, training products, magazines and newspapers, as well as on websites hosted by Durham University and other organisations including social media such as Facebook and Twitter. We may also make video or webcam recordings to promote the work of Durham University to partners, funders or other charitable institutions or for other educational use.
To comply with the GDPR and Data Protection Act 2018 we need your permission before we can photograph or make any recordings of your child.

Conditions of Use:
This form is valid for 3 years from the date you sign it. The consent will automatically expire after this time. We will not re-use any photographs or recordings after the 3 year period.
We will not use the personal details or full names (which means first name and surname) of any child in a photographic image, on video, on our website, in our prospectus or in any of our other printed publications.
We will not include personal e-mail or postal addresses, or telephone or fax numbers on video, on our website, in our prospectus or in other printed publications.
If we use photographs of individuals, we will not use the name of that child in the accompanying text or photo caption.
If we name a child in the text, we will not use a photograph of that child to accompany the article. We may use group photographs or footage with very general labels such as Holiday Camps 2022.

Cancellation policy
If you book any days and then wish to cancel, a refund will only be provided if we can refill the place with another child. This is to ensure we have sufficient staffing for all camps and to minimise late cancellations. To cancel and request a refund please email Phill at who will try to fill the place in order for you to obtain a refund.

If the day you would like to book is showing as FULL, please send an email with your child's name, their age, the day you require and your contact number to us at and we will add you to a waiting list and contact you if a place becomes available.

  • Multisports - (ages 5-12)
    This camp can be booked by day from Monday 25th July to Friday 2nd September

  • Football camp – (ages 8-12)
    This camp can be booked from Monday 25th to Friday 29th July

  • Adventure Camp – (ages 12-16)
    Camp runs from Monday 1st - Friday 5th August, and again from Monday 22nd - Friday 26th August

  • Dance and Drama – (ages 5-8)
    This camp can be booked by day from Monday 8th to Friday 12th August

  • Tennis camp – (ages 5-8)
    This camp can be booked by day from Monday 15th to Friday 19th August
