Go to German Version of Survey Start of Letter of Information You are being invited to participate in a research study that investigates how people with vision loss go about their daily life. The study is conducted at Durham University under supervision of Doctor Lore Thaler, Department of Psychology, Durham University, Durham, UK, Telephone UK: 01913343290, E-mail lore.thaler@durham.ac.uk. The survey was designed in collaboration with Daniel Kish (daniel.kish@worldaccessfortheblind.org). The research team consists of Jack Stevens (jack.stevens@durham.ac.uk), Peter Maxakov (p.a.maxakov@durham.ac.uk) and Nijole Skveres (n.j.skveres@durham.ac.uk), and Doctor Lore Thaler. This project has been given advisory approval by the Applied Psychology Ethics Committee in the Department of Psychology at Durham University. The purpose of this research is to find out what influences the independence of people with vision loss in their daily life. To investigate this, we will ask you to complete a questionnaire that has 45 questions. The questions range from basic questions about yourself (i.e. age, gender), to questions about your vision loss, to questions about your education, work, mobility or social life. It may take up to 30 minutes to answer all the questions. Participation in this study is voluntary. You may skip questions. If you want to skip a question, do not answer it. While this study will not result in any direct benefit to you, it may help us understand what influences the independence of people with vision loss in their daily life and therefore be of some benefit to people with vision loss in the future. Any information obtained from the project will be kept confidential. We will not collect any identifiable information about you. Thus, if you e-mail the survey back to us, we will record your answers, but we will delete your e-mail and your e-mail address. The only exception to this is if you ask us to contact you at the end of the survey. In this case we will record the e-mail address or phone number that you provided. In the event of publication, any data will be identified without any reference to your name or personal information. Thus, if you do provide us your e-mail address or phone number, this information will not be revealed in any publications that might arise from the project. Furthermore, this information will be kept confidential by Doctor Lore Thaler and not be revealed to any third parties. To participate in the survey, please download the file to your computer. Each question of the survey is written in plain text. After each question potential answers are written in parenthesis. You can either indicate your answer by placing a ] or } behind it, or you can type your answer after each question. You can also type additional comments after each question.
